When bursitis occurs, however, movement relying upon the inflamed bursa becomes difficult and painful. Moreover, movement of tendons and muscles over the inflamed bursa aggravates its inflammation, perpetuating the problem.</p>
<p>Bursitis is commonly caused by repetitive movement and excessive pressure. Elbows and knees are the most commonly affected. Inflammation of the bursae might also cause other inflammatory conditions such as rheumatoid arthritis. Although infrequent, scoliosis might cause bursitis of the shoulders, however, shoulder bursitis is more commonly caused by overuse of the shoulder joint and related muscles.[1]</p>
<p>Traumatic injury is another cause of bursitis. The inflammation irritates because the bursa no longer fits in the original small area between the bone and the functionary muscle or tendon. When the bone increases pressure upon the bursa, bursitis results.</p>
<p>Symptoms<br />

Carl Arnold Ruge is credited with having first defined varicose veins as &#8220;any dilated, elongated and tortuous vein irrespective of size&#8221;. The term commonly refers to the veins on the leg,[1] although varicose veins occur elsewhere. Veins have leaflet valves to prevent blood from flowing backwards (retrograde). Leg muscles pump the veins to return blood to the heart. When veins become enlarged, the leaflets of the valves no longer meet properly, and the valves don&#8217;t work. One cause of valve failure is deep vein thrombosis (DVT), which can cause permanent damage to the valves.</p>
<p>The blood collects in the veins and they enlarge even more. Varicose veins are common in the superficial veins of the legs, which are subject to high pressure when standing. Besides cosmetic problems, varicose veins are often painful, especially when standing or walking. They often itch, and scratching them can cause ulcers. Serious complications are rare. Non-surgical treatments include sclerotherapy, elastic stockings, elevating the legs, and exercise.</p>
<p>The traditional surgical treatment has been vein stripping to remove the affected veins. Newer, less invasive treatments, such as radiofrequency ablation and endovenous laser treatment, are slowly replacing traditional surgical treatments. Because most of the blood in the legs is returned by the deep veins, the superficial veins, which return only about 10 per cent of the total blood of the legs, can usually be removed or ablated without serious harm.[2][3] Varicose veins are distinguished from reticular veins (blue veins) and telangiectasias (spider veins), which also involve valvular insufficiency,[4] by the size and location of the veins.</p>
<p>Symtoms </p>
<p> * Aching, heavy legs (often worse at night and after exercise).<br />
 * Appearance of spider veins (telangiectasia) in the affected leg.<br />
 * Ankle swelling.<br />
 * A brownish-blue shiny skin discoloration near the affected veins.<br />
 * Redness, dryness, and itchiness of areas of skin - termed stasis dermatitis or venous eczema, because of waste products building up in the leg.<br />
 * Minor injuries to the area may bleed more than normal and/or take a long time to heal.<br />
 * In some people the skin above the ankle may shrink (lipodermatosclerosis) because the fat underneath the skin becomes hard.<br />
 * Restless legs syndrome appears to be a common overlapping clinical syndrome in patients with varicose veins and other chronic venous insufficiency.<br />
 * Whitened irregular &#8220;scar-like&#8221; patches can appear, especially at the ankles, &#8220;atrophie blanche&#8221;.</p>
<p>
Conservative treatment</p>
<p>The symptoms of varicose veins can be controlled to an extent with the following:</p>
<p> * Elevating the legs often provides temporary symptomatic relief.<br />
 * &#8220;Advice about regular exercise sounds sensible but is not supported by any evidence.&#8221;[8]<br />
 * The wearing of graduated compression stockings with a pressure of 30–40 mmHg has been shown to correct the swelling, nutritional exchange, and improve the microcirculation in legs affected by varicose veins.[9] They also often provide relief from the discomfort associated with this disease. Caution should be exercised in their use in patients with concurrent arterial disease.<br />
 * anti-inflammatory medication such as ibuprofen or aspirin can be used as part of treatment for superficial thrombophlebitis along with graduated compression hosiery – but there is a risk of intestinal bleeding. In extensive superficial thrombophlebitis, consideration should be given to anti-coagulation, thrombectomy or sclerotherapy of the involved vein.</p>

<p>Chronic sinusitis</p>
<p>Chronic sinusitis is a complicated spectrum of diseases that share chronic inflammation of the sinuses in common. The causes are multifactorial and may include allergy, environmental factors such as dust or pollution, bacterial infection, or fungus (either allergic, infective, or reactive). Non allergic factors such as vasomotor rhinitis can also cause chronic sinus problems. Abnormally narrow sinus passages, which can impede drainage from the sinus cavities, can also be a factor.</p>
<p>Symptoms include: nasal congestion; facial pain; headache; fever; general malaise; thick green or yellow discharge; vertigo or lightheadedness; blurred vision, feeling of facial &#8216;fullness&#8217; or &#8216;tightness&#8217; which worsens on bending over; aching teeth, and halitosis.</p>
<p>Very rarely, chronic sinusitis can lead to Anosmia, the inability to smell or detect odors.[citation needed]</p>
<p>In a small number of cases, chronic maxillary sinusitis can also be brought on by the spreading of bacteria from a dental infection.</p>
<p>Attempts have been made to provide a more consistent nomenclature 6 for subtypes of chronic sinusitis. A task force for the American Academy of Otolaryngology - Head and Neck Surgery / Foundation along with the Sinus and Allergy Health Partnership broke Chronic Sinusitis into two main divisions, Chronic Sinusitis without polyps and Chronic Sinusitis with polyps (also often referred to as Chronic Hyperplastic Sinusitis). Recent studies which have sought to further determine and characterize a common pathologic progression of disease have resulted in an expansion of proposed subtypes. Many patients have demonstrated the presence of eosinophils in the mucous lining of the nose and paranasal sinuses. As such the name Eosinophilic Mucin RhinoSinusitis (EMRS) has come into being. Cases of EMRS may be related to an allergic response, but allergy is often not documentable, resulting in further subcategorization of allergic and non-allergic EMRS.</p>
<p>A more recent, and still debated, development in chronic sinusitis is the role that fungus may play. Fungus can be found in the nasal cavities and sinuses of most patients with sinusitis, but can also be found in healthy people as well. It remains unclear if fungus is a definite factor in the development of chronic sinusitis and if it is, what the difference may be between those who develop the disease and those who do not.</p>

This broad definition thus avoids specifying a precise duration for sexual relations and reaching a climax, which is variable and depends on many factors specific to the individuals engaging in intimate relations. An occasional instance of premature ejaculation might not be cause for concern, but, if the problem occurs with more than 50% of attempted sexual relations, a dysfunctional pattern usually exists for which treatment may be appropriate. </p>
<p>To clarify, a male may reach climax after 8 minutes of sexual intercourse, but this is not premature ejaculation if his partner regularly climaxes in 5 minutes and both are satisfied with the timing. Another male might delay his ejaculation for a maximum of 20 minutes, yet he may consider this premature if his partner, even with foreplay, requires 35 minutes of stimulation before reaching climax. If intercourse is the method of sexual stimulation for the second example and the male climaxes after 20 minutes of intercourse and then loses his erection, satisfying his partner (at least with intercourse), who needs 35 minutes to climax, is impossible. </p>
<p>Because many females are unable to reach climax at all with vaginal intercourse (no matter how prolonged), this situation may actually represent delayed orgasm for the female partner rather than premature ejaculation for the male; the problem can be either or both, depending on the point of view. This highlights the importance of obtaining a thorough sexual history from the patient (and preferably from the couple). </p>
<p>The human sexual response can be divided into 3 phases: desire (libido), excitement (arousal), and orgasm. The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) classifies sexual disorders into 4 categories: (1) primary, (2) general medical condition–related, (3) substance-induced, and (4) not otherwise specified. Each of the 4 DSM-IV categories has disorders in all 3 sexual phases. </p>
<p>Premature ejaculation may be primary or secondary. Primary applies to individuals who have had the condition since they became capable of functioning sexually (ie, postpuberty). Secondary indicates that the condition began in an individual who previously experienced an acceptable level of ejaculatory control, and, for unknown reasons, he began experiencing premature ejaculation later in life. With secondary premature ejaculation, the problem does not relate to a general medical disorder, and it is usually not related to substance inducement, although, rarely, hyperexcitability might relate to a psychotropic drug and resolves when the drug is withdrawn. Premature ejaculation fits best into the category of not otherwise specified because no one really knows what causes it, although psychological factors are suggested in most cases.</p>
